Ongoing, Personalised Support

We will provide you with an initial assessment and follow up visits for as long as you need. These visits can be arranged in our clinic rooms or in your home if necessary.

Further support or advice may include:​

  • Aged Care Assessment (ACAT) application support

  • NDIS application support

  • Liaison with your own GP   

  • Medication management & review

  • Ongoing symptom advice and support

  • Rehabilitation following a 'Strengths Based Approach'                     

Sometimes relatives of people living with dementia will encounter difficult and emotional situations where their relative may be experiencing behaviours of concern. 

Working with Multi-Disciplinary professionals allows you to gain knowledge, skills and advice on ways to optimise your Quality of Life.  

A collaborative approach

  • Occupational therapists

  • Psychologists

  • Physiotherapists

  • Nurses

We’ll help coordinate your care, connect you with trusted providers, or work with your current team.
